Sunrise of Five Forks delivers an outstanding lifestyle experience at its senior living community in Lilburn, Georgia. Sunrise is very homey and clean. [name removed], the consultant was very professional and friendly. Decided to use another facility because the cost was a little to high. We found the facility within our neighborhood which was a major factor in our selection processing.

We are very with Sunrise Five Forks, the facility is very nice, the staff is wonderful to my Dad, and my Dad loves the food. He said he doesn't want to leave. Since we put him there, he has improved physically. We all are very happy with this experience.

Sunrise at Five Forks has taken care of our mother for the last 7 months. They cared for her needs in independent living & then in memory care. Each area was set for her specific care for her needs at that time. The caregivers were attentive, professional and very kind. They all knew who we were as family members as well as who our mother was. There were activities in both areas to challenge including the memory garden. When we had concerns I could always count on the directors of their areas to help guide me along the way for her care.

Lots of activities. Care givers treat the parents very well! They seem to really care! This is one of the few places that accept both parents being at two different level of care. One needing skilled nursing and one just needing assisted living. Flip side they nickel and dime everything possible. Increased prices on level of care without discussion. Levels that weren't never brought up when we initially put the parents there. Communication is an item that they def need to work on.

5 all across the board! What we like the most about Sunrise is First is the staff! They're so attuned to the needs of the residents & treat them with such kindness & respect. Plus, there is a sufficient number to maintain a safe, caring environment. Especially b/c my mother is in the memory care unit, I am happy that her needs are always met and handled with dignity. Next, the facility itself is beautifully maintained. It's a very welcoming place from the front door & throughout the building. I should also mention that there are planned activities throughout the day, both in the assisted living & the memory care unit. I want to mention the food: it's exceptional! And residents have choices. At lunch & dinner the meals begin with soup or salad, a complete entre, and dessert. These are some of the reasons I give Sunrise at Five Forks an excellent recommendation

Sunrise at Five Forks has been an absolute blessing for our family! The care team is incredibly compassionate and attentive, truly knowing and valuing my mom as an individual. Over time, they have become more than caregivers—they have become like family, treating her with the same love, patience, and kindness we would. The facility is always clean, the leadership team is incredibly supportive and communicative, and the entire staff goes above and beyond to create a warm, home-like environment. Knowing she is in such capable and caring hands gives us incredible peace of mind, and we are so grateful for the exceptional care they provide! Words cannot fully express our gratitude for the incredible care my mom receives at Sunrise at Five Forks. The combination of deeply personalized attention, a loving and dedicated team, a beautifully maintained environment, and strong leadership makes this a truly exceptional place. Poor accounting system. Bookeeper can't explain billing. Poor customer service. Calls not returned from both Accounting and Executive staff. Have been waiting on my $[removed]+ refund for months. Avoid this facility at all costs.

My mother has been a part of the Sunrise community for several years, and every time we visit, we’re continually impressed. From the professionalism and friendliness of the staff to the quality of care and even the meals provided, everything reflects a commitment to excellence. Our mother has been a resident at Sunrise Assisted Living for 2.5 years, and the experience has been outstanding. We initially moved her from another assisted living facility in a different metro area, and the difference was apparent immediately. From the moment we walked through the doors, we were struck by how genuinely friendly and welcoming the staff were—a warmth that has remained consistent throughout her time here. The level of care our mother has received has been exceptional. The meals are of excellent quality, and the staff maintain her room and the entire facility in immaculate condition. The parents are in two different physical states. We needed skilled nursing and assisted living. Not many places offer both on one facility. I will say it is extremely expensive and but can be because there aren't many other options

This place is special. Excellence begins at the top- [name removed]is a kind person that genuinely cares about the people there. This can also be said about the staff. [name removed]is the best! She is conscious about the meds and how they are given. From the people that greet you at the desk, like [name removed],and [name removed], always a smile and a helping hand. There are many like this and I can’t remember their names. This is a real home away from home.

Each staff member has a smile on their face and a greeting when I see them. They are very kind and loving to Mom and Dad. It's a great comfort to know that they are well cared for and loved. The care of the residents is top priority.

Staff is excellent and friendly. Food was really good. Room was nice and overall floor plan is good (not a long walk for anyone with mobility issues). My only negatives are minor and mom was only there for about 2 months so we couldn't settle into a routine. 1) billing is based on a daily rate so it's hard to budget for fixed income and the online bill payment system from corporate in KY is confusing. 2) mom used Express Scripts Pharmacy for her military discount and it appears that the local pharmacy vendor Guardian filled a couple of prescriptions intended for Express Scripts. (I haven't even started working on the dispute with them yet.) Overall I would still STRONGLY recommend Sunrise for your loved one.

My father lived at Sunrise for over 5 years. He received excellent care and thrived. This facility provides fun social activities and nutritious meals. My dad had many friends at Sunrise and very kind caregivers.

What we like the most about Sunrise of Five Forks is the level of care that caregivers provide for [name removed]. The quick response from administrators when I have questions about administrative matters or questions about [name removed] care. The facility is well kept and staff in [name removed]area keep the area clean.

Our experience was very good. My father was there for over a month. The staff was very compassionate and professional. The food was well done. Activities were appropriate and abundant. The garden area was very nice. In conclusion, I would recommend this facility strongly.
