The Brookdale HealthPlus Care Manager (RN) can serve as a medical advocate for residents and manage a variety of needs. The Care Manager can coordinate care with residents’ doctors and specialists, organize care transitions, schedule follow up visits, and more. Annual wellness visits, immunizations, and health screenings are also provided. Plus, residents can receive support for chronic conditions, like diabetes, high blood pressure, and congestive heart failure.</p> <p>If you need assistance with transportation, chores, or meal preparation, we can help. We provide three homemade meals each day, along with a variety of snacks and beverages. Additionally, we manage your home maintenance and mundane chores. You’ll no longer have to worry about washing dishes or mowing the lawn.
