Here, care coordination is overseen by a registered nurse. Preventative measures such as annual wellness visits, immunizations, and health screenings, as well as support for chronic conditions such as diabetes, high blood pressure, and congestive heart failure, are offered to help avoid trips to doctors’ offices, or unnecessary emergency room visits and hospital stays. Your wellness is supported through collaborative care partnerships, and we’ll make sure your chores, home maintenance, and laundry are all taken care of.
