A Place for Mom has scored Premier Senior Living, LLC with 7.9 out of 10 using our proprietary review score based on 77 reviews in the last 2 years. Over all time, Premier Senior Living, LLC has 439 reviews with an average "overall experience" of 4.2 out of 5.
Do thorough research before making a decision. This Brookdale facility has had MANY substantial management/administrative leadership issues since mid 2015. As a result, there have been MANY staffing issues, food issues, dining room/staffing issues, etc. Excessive fire alarm evacuations very early in the am. (Well over the number mandated by state.) Lack of response to resident accidents (emergency in room pull cords), or in any timely manner. This facility has now a new Director just this year. Based on the history of problems one would think that their priority would be to establish goodwill with the resident population to attempt to mend damage. Not the case! Professionalism seriously lacking. Not available. Total lack of follow through with families on standing issues despite (numerous) attempts to get resolution. Brookdale cannot afford to continue ignoring things. Residents have confided their negative impressions and accounts re: this facility and upper management. On a good note, there are some terrific people on staff who are dedicated, responsible, and professional.......Not enough to balance the lack of : transparency, genuine care and concern, and resolution/results to affect change.

Upon entering the building there was no one to greet us. I had to walk aroudn to try and find someone to assist me. No one left their office to come out and see if we needed assistance. I finally got someone to help me so I could find out what room my family member was in. I proceeded to her apt. and came across a strong odor, the odor continued to be there when I left the building as well. Staff not very friendly, when walking past them, nor were they when they came in to give medications. The bathrooms are very spacious and the room has alot of storage, carpet was not very clean or appealing. My family member has an aide but not once did another aide from Ivy Hall come into to check on her the whole time i was there. She has to still pay for their services even if she has an aide, but they should at least come in to check in on her once in awhile. Ivy hall is adequate for the price but if i had a choice I would have put her somewhere else. The parking as well is a diaster, they shouldn ot allow people to work there park and then leave no spaces for family members.

My mom has lived here for a while. It was nice when we first placed her there several years ago, but now it is terrible. All the employees are new. Not sure what is going on, since there is no one who will explain why everyone my mother was used to and liked, is now gone. It used to have a warm, cozy feeling now it seems cold and uncaring. Staff used to be kind and friendly, now they don't even speak to you when you enter the building. My mom looks disheveled and the staff makes excuses that she didn't want to bathe, wasn't in her room when they came by to help her, etc. Mom complains there is nothing to do, no activities. Room is not very clean and mom complains chairs in common areas are often wet from urine. No exercise equipment, so if your loved one is capable of moving around and wants to exercise, no way to really do it, short of roaming the halls. Food is pretty good.

Have been contacting the Oaks following our 14 month stay to be reinbursed our $[removed] security deposit. They just pass the buck from one to the other saying they never collected a security deposit. We have it written on our contract and were advised by the initiating marketing agent this would be reimbursed upon our decision to leave. Honestly, we were delighted to have a beautiful home and a future that would carry us both to the end of our lives, the buy in seemed well worth it. Exactly 11 months after this the owner sold the assisted living and the skilled nursing care??? Well, where do we go from independent living—nowhere!! Realizing $[removed]/month was a little high for renting a home with no future. Actually, we didn’t expect the return of the $[removed] to become such an issue since this is quite minimal compared to the investment we have lost. Not one owner, proprietor , marketing, or senior advisor will answer our request, like we don’t exist. Cannot recommend the Oaks now!!!

My sister was admitted before the hurricand, but since then, I have NOT been able to get hold of her or the acute care nursing staff, can't any number to call except [removed]. Both give me nothing but a busy signal like it's off the hook. I called state agencies, but all say they can't do anything One suggested I call the police to do a welfare check. Health are facilities don't close so someone HAS to be there. Tomorrow, try again and take further measures. I even got her a new phone and she used it to call me on it, the only way I could talk to her and that only happened once.

The owners, management & leadership of this company & facility DO NOT CARE about maintaining standards of care, appropriate policies, procedures> Staff aren't adequately trained. They hire & pay for the cheapest possible staff, totally unqualified. Anyone qualified quits within 2-3 months the management is so bad. The food isn't prepared by qualified staff & feed residents processed garbage like chicken nuggets & bologne & cheese sandwiches for Thanksgiving dinner. They NEVER did a care plan for my loved one in almost 3 years, failed to keep her clean, maintain hospice records. There was massive theft of her belongings, clothing, bedding, towels, shoes, gifts by other residents & staff. Several members of staff don't speak a single word of English? How were they possibly hired? They cannot communicate with residents, coworkers or management? Substandard care in every possible way. Never place a loved one in this dump.

I was mortified by the lack of professionalism. This facility had my mother in there care during a respite stay for an excess of 3 days in her chronic condition. They had not even done the bare minimum by monitoring her vital signs after receiving updated med list and a full physician evaluation report after having ordered additional medications along with medications that were provided for her stay were never given to her. After speaking with their nurse director about this issue he retailed and sent my mother to the emergency room without all of her personal belongings and none of her medications. He also threatened to report ME to aging services after I asked to speak with the owner in the corporate office. He proceeded to provide me with a corporate number for employees which was intentionally inconclusive information. I will not recommend this facility to any family interested in a safe place for their loved ones

Our family had two main issues with the way Harbor Cove was ran - first they basically would bring all the residents into the main hall and plant them in front of the TV, and that was all they did. Secondly they had a couple of residents that had no business being there, they were mentally impaired, very loud, nasty and just disruptive. That had caused a lot of problems

This was definitely not the place for mom. Not only is the building disgusting but the executive director and the wellness director was also disgusting(RUDE). They have not had a housekeeper in months and the building shows it. The place smells of urine as soon as you come through the door. They are always short staffed with only 2 people to care for 25+plus residents. My moms laundry was always piled up or lost in the facility with another resident Wearing it when I come to visit. The visit is never peaceful there are always alarms going off and people yelling. One day I even saw a combative resident strike another resident and a worker that is when I decided to report my concerns to management they were ignored and i was brushed off. I do not recommend this place to anyone looking for a great SAFE place to put their love ones. When I did the tour I was fooled because they put on such a nice act. But I began to see the truth once visiting my mother on regular bases. Hope this helps.

Mother in law was in this facility for 5 days. We prepaid for 30 days. She was left in her bed with soaked bandages for 4 days. Her call button was broke. No one paid attention. She was removed after 5 days and the refunded a small portion of our prepayment but refuse phone calls asking for the 25 days she was not there to be refunded.

I waited 25min before I was toured. The place has a strong odor. I was greeted later by Ms. Wise. The place is more than an assisted living. As I was waiting one resident kept getting out of the building. This was not a safe feeling for me. The atmosphere was not warm and inviting. I want my mother to feel safe and secure. Touring the facility seems like the facility is more like a nursing home. One resident was walking around soaking wet the time I was there. Before I left I wanted to know the name of the Lady sitting in the office. I was told it was the Administrator. The Administrator actually let me wait 25 min on someone else to tour me. I was not happy with this place. I went to Lexington Gardens and felt that warm feeling. I was introduced to staff and they all were smiling and friendly. The Administrator gave me a tour and the entire staff was helping residents. This is what I needed to see and made my decision easier.

If your loved one is going into memory care and has any issues at all, even the smallest little issue, they will call an ambulance and send them right off to the hospital. They want their memory care people to walk around and sit in a rocking chair and not cause any kind of behavior. Any behavior at all, and they send them right out. And then they charge you while your loved one is in the hospital for over a month. After being back from the hospital for an hour and fifteen minutes, they then kicked him out. The day-to-day staff was pretty good, but the director was terrible. The facility was clean. But their utter lack of understanding and ability to deal with dementia behaviors has been completely damaging to our family. This is not a good place for memory care, and I wouldn't want anyone to go through what we went through there.

My mother was put in a filthy room with a toilet that did work. There were cockroaches and mold. They left her in the hall way for hours and ignored her. They have a terrible reputation and have numerous citations from the health department and medicare violations. Staff would get angry when she was in need of something and generally ignored her. Do not put your loved ones here.
