I would want others to know that if your loved one has dementia and needs care such as bathing, changing diaper, guidance, you are screwed. He was here for 30 days and destroyed him, I got charged almost 20,000 dollars and I have not gotten a dime back. He had to go to the hospital due to the little care he received. They were not giving him any of his medications. They were not helpful at Commonwealth Senior Living at all.

The Resident Care Associates (RCA) can be very aggressive when providing bathing care. Baby wipes are often used to wash instead of soap/water and wash cloths. Residents may have their own items provided by family members but they are not typically used. Residents are yelled at for trying to help remove clothes when it is time to change and when trying to exercise their independence. Residents are yelled at when they're on the phone with family members. When responding to complaints that the water was too hot, the RCA stated "She can take it." referring to the resident. While waiting for the shower water to warm up in another room, the resident was splashed with cold water several times by the RCA. Another resident was asked whether he wanted to take a shower because it was on the schedule. The resident asked when the last time he had a shower was and the RCA replied, "Remember you didn't like when I gave you a shower last time. You don't like it. Do you want to take a shower?" The resident responded, "Well, in that case, no." The RCA logged his response as a refusal. Residents were disturbed well into the night after they've fallen asleep for vacuuming and laundry, sometimes even showers that are supposed to be done prior to bed are delayed. Care provided at this facility is very different from what you see in the promotional videos or even during daylight hours. If your loved one lives here and requires any degree of assistance to help change clothes, bathe, or eat, be present often and see how they react when staff provides that "care." The food is standard elementary school cafeteria style, not the chef inspired meals advertised and charged for. Complaints have been filed on behalf of several residents.

I placed my aunt in Addington on Feb 22nd and it has been the most horrible experience so far! The fact my aunt lives in Michigan and I live in Kentucky has been highly stressful because my aunt's needs are not being met and when reported, staff members say the right stuff; but they do nothing. Within her two week stay, my aunt has fallen at least four times; while being assisted by aids. The most recent fall of March 6th, left my aunt's face badly bruised and fingers on her left hand in intense pain; no x-rays were taken. I plan to seek legal remedy to flag this facility as lacking and incompetent in providing senior care. I regret placing my aunt in Addington Place.

Addington Place of Grand Rapids is not as advertised on their website. The meals are not “home cooked” unless you call heating up foods from Gordon’s Foods home cooked. The meals are way time big calorie and there is no room for change! I frankly don’t know how the diabetics manage unless they just increase their medications. Experienced, dedicated staff that offer personalized care with compassion is very small in number. Most staff have little to no experience and are “trained” at the facility which sometimes means putting staff on that has no training. Most of the staff seem indifferent to compassion. The place is always short staffed and the turn over rate is extremely high. I cannot keep up with staff names since there are always new staff. Sometimes there is 1 staff member on duty who is suppose to give care to 15 or more people for almost the whole shift. This may include giving out medication, getting people ready for bed, and answering the buzzer which tells staff a person needs something like going to the bathroom. The “years of experience” only tells you how long the place has been open, not how many companies have managed them and not that the staff have years of experience. Your loved one may not get their medicines like they are suppose to. Medicine is sometimes not given for no good reason and sometimes they don’t get it because staff don’t always call the pharmacy to have more medicine before the refill runs out. Sometimes staff write that they have given the medication when they have not. My friend did not get her heart medicine for 12 days, another time her blood thinner level was not checked for four months and nobody noticed. She could have died her blood was so thin. Another time a different medicine was not given for four days. There is so much more but I am afraid you won’t read this if it is too long. Be sure that you want your loved one to experience these possibilities. They are real.

The review before this is so right on. Was hoping the new management would make a difference! And it is so traumatic to move a parent in memory care so these reviews are the only tool that families have to warn others & push for change. Concern sheets & meetings mean nothing when the director does not make sure personnel have work ethic & supervision. Example: so -called tour night -Thursdays 4-7 consists of managers & booze meeting with young friends in resident's cafe while residents are pleading to go out and walk in the unkept courtyard! Unreal!

Addington Place 68th St Gand Rapids DO NOT GO THERE. rooms are a nice size. The staff, nurse and Director are all nice and give you the right answers. (many new directors) Staff Doctor does not do what he is suppose to. They do not properly care for the residents. My guy was there for a little over a year and was hospitalized multiple times for dehydration and infections. They did not keep a cup with a lid and straw next to his bed. They gave him a 4 ounce open cup that of course would go down his shirt when he tried to drink. Many times over 2 weeks with no shower. His muscles were lost by time I got him out of there. Instead of taking time to get him out of bed and walk him to the meals, they put him in a wheelchair and that is where he stayed. No activities as promised. Keep your loved ones safe and do not use this place

My wife was a resident of Prestige Commons for a short time under hospice care. I have no problem with the staff or care at the facility its the management that is the problem. I would advise anyone going into this facility make sure you read very carefully any papers you sign. I was never given a copy of the papers I signed or had any explanation of their refund policy. I paid for a full month $3720 & my wife passed away on the [date removed]. I was told I would get a refund for the remaining days of the month. After waiting a month I was told their policy had changed & no refunds would be given. My advice to anyone thinking about going to Prestige Commons be advised you could loose up to 4 or 5 thousand dollars. I thought this place was different the all the others but it only about the money..BEWARE [Name removed]

Do NOT place your loved one at this facility. My father’s last undocumented fall resulted in him no longer being able to walk, he has trouble speaking, and the staff who should have taken care of him and been attentive to his needs say what you want you to hear to your face and then work to cover up staff inadequacies on every level. My family and I have have filed a complaint with the state. My father was neglected which resulted in his demise, medication withholding which I have records and proof, medications “misplaced”.. this facility should and will be audited to the fullest extent and I pray that those caring for staff are fully reviewed, drug screened , and nursing leadership and some med tech REPLACED My father spent every dime of his retirement given this this facially to take care of him. My mission is to make sure that they are investigated so that no other resident ends up like my father. This place is awful. My sister was under their care and it wasn’t until she passed that we found out how ingored she was. Received very little care. Incontanence was handled by making her wear 2 depends to hold her urine longer, so they weren’t bother with changing her. She was given the. Does to the doors so she could go to other units without assistance. She was found fallen down a hill on East Paris with her face smashed into an iron sewer grate. The home didn’t even know she was missing u til they were notified by the person that found her. Don’t put your loved ones here.

Those who can live independently in assisted living at this facility are fortunate. Those who live on the dementia unit (and those from the other 2nd floor who should be on the dementia unit or aren't able to live independently) need action to insure the care and safety they deserve. The investors of this community need to step-up and support management with trained personnel to give these residents the care they desperately need and deserve. Dementia-residents sitting in the dark, watching action movies from 7-10pm without an aide present, is not an appropriate-nighttime activity for alzheimer residents. Workers hanging out in groups of 3-4, during this time , instead of interacting with those who need attention, is not resident-centered care. Aides who do not speak appropriately with dementia residents are not educated in alzheimer-care. Poor smelling hallways and unmade beds is not basic-care. Things are the worst after 4pm & on weekends. Noticing more and more vacant rooms on the third floor. Action needed now!

Some days there are good things at this home but basic care is not consistent. I left the 3rd floor today and an elderly visitor complained to me in elevator. She found a friend from church she was visiting, in unacceptable condition, as well as her bathroom, due to incontinence. This was a repeated situation for her friend, that I have also experienced with our loved one. This lady was so distraught, because she has found her this way before and talked to aides about it. These are residents who need specialized care and are not being attended to by sufficient, trained staff on an Alzheimer's unit. When you visit, to check on friends and loved ones, be sure to check their rooms. If they are often in their room alone, bed unmade & without fresh hydration chances are they haven't been cared for very well that day. Representative, director -of -operations, owner and management, we care enough about the dignity of the elderly, to give this review. This home was chosen at high cost to give exceptional care, not substandard care. These residents deserve better.

Really wish it wasn't so, but this care home continues to decline. There is no supervision of employees that are caring for the elderly. The minimum is done for the residents. That is why you see residents with doors shut and care aides sleeping, watching tv, using cell phones, etc. Before putting your loved one in this facility visit often, unannounced and especially on weekends. When you give a review like this, the representative for First & Main asks you to contact the executive director. The executive director says his hands are tied for staffing improvement. This is the 5th ED in 2 & 1/2 years and it is the same story. You really can't blame young, unqualified, undisciplined, underpaid and unsupervised aides. There is only one hope-new ownership that will give the executive director the tools needed to make this assisted living and memory care home the vibrant community it should be!

Staff is untrained and uneducated. It feels like they grab anyone off the street. Staff call in sick all the time and are not covered so residents go unattended. If there is an issue it is ALWAYS the residents fault and the family must solve the issue as they have NO experience or interest in assisting the elderly. They behave as if they are doing you a favor letting your loved one stay there. You will pay for all the promised services and receive only the fraction they are willing to squeeze out. Oh and I know the response will be call [name removed]-who will say he is looking into it and will never follow up or solve the problem. The care team leadership acts superior and can't be bothered with even the smallest issue because once again if there is an issue it is the families problem not theirs. This place is a disgrace and we are so happy to have saved our loved one from their disaster.

Be very careful ! You have to stay on top of the staff and make sure they are taking care of your loved ones. The staff gives the appearance they are attentive when you are there. You need to check on your loved one EVERYDAY to make sure they are properly cared for. There was food behind my loved ones bed, they give them medication to make them sleep ALL day. The Assistive Living for our daughter has gone from very nice, four years ago to horrible. The aids use vulgar language. Have earbuds in their ears while "attending" to our daughter. They can't hear her and have a don't care attitude. When she complains to the management it comes back to her and the aids threaten that they won't help her or work with her etc. There are employees who refuse to care for her. I call it bullying. It's difficult for me to see our loved one go through this. The stress she is going through is affecting her health. On one visit, my husband and I noticed fecal matter in her sink and a pan with dirty wash water and cloth on her computer desk. Filthy.

As family members of a loved one at this home-for-the-aged, we give our lowest rating possible. The owner, regional director and even the state, have refused to take action to protect our loved ones. Sufficient personal & medical care is unacceptable. Trained & professional staffing is barely existent. Social activity is poor. Many residents have left or are planning to leave this private-pay-home in the last couple of months. This facility is quite new & very modern with opportunity for a wonderful place for the elderly. But the sad part is basic care and social activity is compromised. Staff do not show up for work and constantly being replaced. They are not qualified or given proper training. Social life is very sparse and many residents stay in there rooms even when activities are scheduled. Residents & families keep bringing up these issues but management has no interest in resolving issues. Sad to say broken promises continue after 2 years.
