The representative from Oak Park Manor was very friendly and easy to like. We were somewhat concerned during the tour, however. The building is still under construction. The staff seemed very limited for patients in my mother’s condition. Overall, we did not believe it to be a good fit.

We were not happy with the care from this community. We had to move him due to issues he had. They did not want to assist us on the financial side. We had no idea we would be asked to leave. The staff were nice and caring and tried to assist his needs. We did like the menu. I could not recommend this community to others

I toured this facility and was hopeful since it is close to home. The staff was friendly and helpful and the facility is currently adding a memory care unit. Facility is dated and is being updated in some necessary ways, floors being converted from carpet to wood, etc. Residents seemed happy. We requested an assessment and we did not receive a call regarding how that had gone for more that 24 hours. We were under a time constraint and the facility was aware, their delay was very stressful.

Most of the caregivers and med techs are caring and do try, but the management is definitely questionable. One woman in particular was always pretty standoffish, and didn't seem to really care about the elderly people there. Not sure why she's in this line of work. There were issues brought up that were never addressed. For one, I felt like my mom was overmedicated from the moment she arrived. She went from having conversations, to not even able to wipe her nose, to being fully bed ridden. My mom's bathroom was never really kept all that clean. I had to close the door every time I visited. And anytime I had a question or concern, this particular person was defensive and acted as if I was bothering her. Also, I question some of the young people this woman hired. And anytime there was a good med tech, or aid, they'd soon disappear. The turnover is high here, so even if you get to know the people helping your loved one, they will be quickly moved to another area, or leave all together. I feel like this place has a lot of potential if it had better management.

Karen C. from A Place For Mom was extremely helpful. I found this community and after our in-home assessment we were told my dad was good to move in. Our first day was stressful, there were no doctor orders ready. This may be dues to management change and resulted in miscommunication and no organization. Things did not end well for Dad and the communication to us was not a priority. Overall they were tough on Mom and the community needs better training and better communication between all the staff.

I wish this would let me post pictures of the filth we found our dad in when we went to see him. 90% of employees were fill ins from temp agency. Dads new bottom dentures were lost 2x, he died while we were there without them being found, and they weren’t paid for yet. The Director blamed resident “Secret Shopper” for dads jeans, pajama bottoms, belts, underwear, socks, new pillows and 2 pair of slippers that were constantly being taken or not separated on laundry day. She was told they weren’t responsible for lost personal items. His wife found him naked from waist down with dried feces on his butt after dad had fell the night before. It took a scraper to scrap up the 2.5 week old dried Ensure from his bedside floor. Dad had spilled that before his last fall and hospital visit. Staff told us they were told keep their mouths shut and do their job. Director lied to my face about dads missing blanket. It was later found. Dad didn’t want to die there, unfortunately he did.

It appeared they were short staffed a lot, which I was told should NEVER be the case because they have resources for staff. You are supposed to have 24hr access to your loved one however, there was more than one instance where after the receptionist went home, no one came to the door when someone (including my mom's hospice nurse) came to see her. My mom fell a lot, and there did not seem to be a plan for how to prevent her falls. Again, on more that one occasion I was told, "we went into your mom's room and found her one the floor." One of those times, she had his her head. Lots of excuses but no solutions. I could go on but I would recommend contacting the Deputy State Long-Term Care Ombudsman before continuing to recommend The Rawlin.

Very high caregiver changeover. Phone calls often unanswered because there isn’t enough staff. Staff is friendly. Director doesn’t follow through on concerns. Activity sheet looks busy and full, but if you look closely you’ll hardly ever see much activity other than sitting in tv room. General hygiene has to be requested (shaving, teeth brushing) often or it gets skipped unless you speak up often. Clothes go missing constantly. PT/OT services are a joke. Room is often in need of vacuuming and dusting. If you’re looking for an expensive place with friendly caregivers but basic care of feeding and not much activity other than television, this is the place. Finally moved our loved one out to a facility where the director really runs the place and knows what’s going on day and night and where activities aren’t just on a calendar to look good.

We toured here when we were looking into options. It was right across the street from the hospital so it would have been convenient. However the residents here all seemed to sort of be in zombieville. It gave us a good contrast of what these places could be. It was not a very attractive option.

My mother recently moved out of Waterhouse Ridge after two years. It's a newer, well-furnished, and clean facility that's conveniently located just of HWY 26. What started out as a positive experience devolved over-time into a truly awful situation. Over the two year period, we witnessed a near constant revolving door of young and under-trained care staff, making it challenging for my mother to establish rapport and have consistency in her care. During this time, there was also significant changes in leadership, nursing staff, etc. and in our opinion, the current leadership as of January 2019 is the worst so far. An example of this lack of leadership--over the past couple months leading to eventually pulling our mother our of Waterhouse Ridge, the executive director never once reached out, called, responded to any of our emails, including once we eventually gave 30-days notice. For that matter, no one at the Colorado-based management company did either. While there are some really great and caring staff there, we can not recommend this facility at this time.

We toured the facility and found that others provided better activities. The caregivers seemed unprofessional. We saw a group smoking in the small lot before coming inside. We toured a "neighborhood" overall seemed nice. There was food on the floor and one caregiver ignoring a woman in a wheelchair. The food looked so so certainly not home cooked as they suggested. It was sunny out and Mom wanted to see the garden they have outside but the courtyard doors were locked. I asked a caregiver and she said sometimes they lock them even on the nice days. Overall the tour was poor, I would not recommend unless your parent likes sitting alone for hours.

Took my 1st tour about a year ago. No ramp to get in. 2nd tour month later and had conversations re: Medicaid contracts. The Esther stated that they were applying for their Medicaid Contracts within 6 months. I paid my $[removed] deposit and picked my choice of room. Fast forward ... NO INTENTION OF CARRYING OR APPLYING FOR MEDICAID CONTRACTS. FACILITY OPENED UP OVER 1 YEAR LATER. DEPOSIT AND RENTAL COSTS INCREASED BEFORE THEY ALLOWED TO MOVE IN. BASIC STUDIO COSTS $[removed]. Avoid this bait and switch method of operation!!!!!!!!!

My loved one lived at Iuditas and we had a difficult time getting the care plan updated on a regular basis or even followed. Night staff would sleep. Changes in medications were not tracked or communicated and it was all one big mess. Management was a nightmare to deal with when there was any. I moved my loved one out. The care staff were friendly and tried - but lacked knowledge and oversight.

I’ve had my parent in this facility since it opened and I cant express what a mess this place is. They have gone through 7 executive directors and interim directors in a year and half, with their last one quitting without notice. They have no activities! Ive seen a woman color. I’ve seen the Maintenance man work as a caregiver and a cook, changing briefs, and giving showers and cooking in the kitchen more than the cook. They’ve had 3 management companies come in to try and fix things. I was told their nurse that had been there since it opened finally resigned because of the upper management issues and I never see a nurse on site anymore which is concerning because the staff does not always seem knowledgeable. The maintenance man left a few months ago and who could blame him! Residents miss Dr appointments a lot saying they forgot to set up a ride. Ive seen enough now and I will be looking elsewhere to put my parent and if you are considering this place DON’T. There are better places
